Our Programs

Overview

Northern California Rehabilitation Hospital (NCRH) is a 90-bed hospital specializing in Long Term Acute Care (LTAC), Transitional Care (TCU), and Outpatient Rehabilitation.

In partnership with local hospitals, we have determined there is an ongoing need for both Long Term Acute Care and Transitional Care in our region. In fact, NCRH is the only LTAC facility between Folsom, CA and Portland, OR. We serve as an important resource to families struggling with the need to travel in support of their loved ones as they recover from surgery or cope with a long term illness.

Our LTAC programs include a comprehensive pulmonary protocol in which a Board Certified Pulmonologist and Critical Care physician oversees this program as our Medical Director. Another service line component is the Complex Wound Management Program. A multidisciplinary team with experience in wound management and specialized training direct the care.

In addition, our TCU team is focused on the post orthopaedic joint procedure patient requiring rehabilitation.

Long Term Acute Care

LTAC's serve patients with medically complex conditions requiring long term hospitalization in an acute care setting. Many of the patients are admitted directly from short stay hospital intensive care units or medical/surgical floors with respiratory ventilator dependent or other complex medical conditions.

Licensed as an acute care hospital, LTAC's differ from chronic care settings in that they focus on treating critically ill or high acquity patients in an intensive way using specialized treatment programs geared to the patient's illness and requirements. The goal is medical recovery and return to home and family. LTAC patients have an average Medicare length of stay at 25 days or greater.

Transitional Care Unit (TCU)

The Transitional Care Unit (TCU) is the bridge between the acute hospital setting and home care for patients with a wide array of medical needs. Our setting provides an expanded continuum of services not typically provided for in Skilled Nursing Facilities

Outpatient Physical Rehabilitation Program

Our Outpatient Neurological and Orthopaedic Physical Rehabilitation program offers hour long one-on-one evaluations and treatments with a licensed therapist or assistant. Our professional staff emphasizes self management skills and home program instruction with the potential to progress to an independent gym or pool program in our facility after discharge.
